We Ran the Numbers: Health coaching changes the risk equation

Article Summary

Preventing 10,000 low-risk members from getting worse can matter as much as helping 1,000 high-risk members improve — most benefits strategies only measure one side.

Most ROI conversations around health coaching focus exclusively on high-risk members. Personify Health’s Insights Lab found coaching works across the full risk spectrum: high-risk coached members improved across blood pressure, cholesterol, sleep, and more — while 60–70% of low-risk coached members maintained healthy biometric status year over year. A 5pp blood pressure improvement alone saves $500 per employee annually.

Supporting and managing a healthy workforce sounds straightforward. Keep people who are unwell from getting worse. Help at-risk members improve. Simple, right?

Except it isn’t. Because the members who are already healthy? Without the right support, their health doesn’t necessarily hold steady. It moves. And often, not in the right direction.

That’s the half of the population most benefits strategies miss. And it turns out, it’s just as important to your health engagement strategy and your bottom line.

Something for everyone. Never the same thing for everyone. That’s how we think about personalizing and improving health — and why our Insights Lab data shows results on both sides of the equation from health coaching.

We measure success across the full continuum: That means supporting members at every stage of their health journey – helping some lower risk and helping others stay healthy in the first place.

The gap many benefits strategies miss

At scale, preventing 10,000 members at lower health risk from moving into higher-risk categories can matter just as much as helping 1,000 members at high-risk improve. Yet most wellbeing strategies – and most ROI conversations – focus almost entirely on the latter.

That’s the gap. And it’s exactly where our coaching model was built to play: to meet people where they are on their own health journey and help them get to where they want to be.

What our Insights Lab found

The Personify Health Insights Lab (our team of data scientists, health economists, and actuarial experts) analyzed coaching outcomes across our entire book of business.

The findings were clear and hard to ignore: health coaching changes the risk equation. It’s a population risk management strategy. And when it’s fully integrated into a personalized health platform, it works on both sides of the equation simultaneously.

Two findings stood out

First, among members who started at higher risk, those working with a Personify coach improved at measurably greater rates than their non-coached peers across blood pressure, cholesterol, triglycerides, heart health, preventive care, physical activity, and sleep. That’s a wide range of health domains, and coached members moved the needle across every one of them!

In high-risk populations, the coaching advantage reached as high as +9 percentage points in sleep improvement and +6 points across multiple clinical domains.

👉 The main takeaway? The Personify wellbeing & health engagement platform delivers results. Coaching accelerates them.

And second, among members who started at lower health risk, 60–70% maintained low biometric risk year-over-year, and 70–80% held their ground across key Health Risk Assessment domains.

Staying well is an outcome, not a guarantee. Without the right support, health tends to move in one direction: toward higher risk. That makes maintenance an active strategy, not a passive one.

So how do we make it happen? By making every member feel seen. Our coaches don’t deliver generic advice – they build real relationships, learning what motivates each person and meeting them where they are on their health journey. That continuity of support, combined with personalized tools, Digital Journeys, and science-backed behavior methodology, keeps members engaged long after the novelty wears off. The result: healthier decisions, sustained over time, that quietly protect your population’s risk profile.

👉The main takeaway? Personify coaching keeps lower-risk members where they are: healthy, engaged, and moving forward.

Small shifts. Real dollars.

The clinical improvements coaching produces lead to better health outcomes and lower healthcare costs (that’s our “why” after all.)

Industry research shows that a 5-percentage-point improvement in blood pressure control is associated with $500 per employee per year in savings. A 1% reduction in cardiovascular disease risk can save $1,224 per individual annually. And more than half of catastrophic claims exceeding $50,000 are tied to a missed screening in the prior year – meaning even modest gains in preventive care utilization can help employers avoid their most expensive claims.

We move these numbers. Consistently. Incrementally. And at scale.

Why the Personify Health difference actually makes a difference

Real outcomes don’t come from checking a box on a benefits checklist. They come from how coaching is built, delivered, and sustained – and why we don’t treat coaching as a bolt-on or outsourced add‑on.

They come from people. People who show up every day to change lives. The people who truly put the person in Personify.

Meet Lauren Freedman, NBC-HWC, CNC, CIHC – one of our live services health coaches. Lauren believes in leveling the playing field, filling in the gaps, and showing up for members who feel overlooked or overwhelmed by the system.

“Helping people get back in the driver’s seat — that’s the biggest joy in what I do.”

Our coaches aren’t contracted out. They stay with members over time to build the trust and accountability that matter most when life gets complicated. They’re NCQA‑certified, with many holding advanced degrees. And because they operate inside our integrated platform, not alongside it, because coaching doesn’t stand alone. It activates everything around it.

Digital Journeys. Partner programs. Tools members already have access to but now use more fully.

The impact compounds. Coached members engage 2.3× more overall, complete 4.2× more Journeys per year, and are 3.5× more likely to explore partner programs. That’s not a coincidence. It’s the result of a coaching model designed to elevate the entire health experience – not just one piece of it.

What to do differently starting now

If coaching is in your benefits package – or you’re evaluating it for the future – here’s what the data says to do: Position it as a risk management strategy, not a wellness perk.

  1. Build the business case around long-term risk trajectory, not short-term participation metrics.
  2. Map it against the programs you already invest in and structure it to close utilization gaps from day one.
  3. And measure success at 12–24 months — because the greatest value of coaching shows up in consistent, incremental improvement and high maintenance rates that quietly reshape your population’s risk profile over time.

The bottom line

Managing population health isn’t just about helping people get healthier. It’s also about making sure they stay that way – and having a partner who’s built to do both.

That’s what Personify Health coaching delivers. For members at higher health risk, it drives real clinical breakthroughs. For members at lower health risk, it protects the progress they’ve already made. Together, those two outcomes are what a long-term risk management strategy actually looks like.
