Why This Role Matters
Chronic conditions like diabetes, CAD, and CHF don't manage themselves — they need someone who can translate clinical data into a plan a real person can actually follow. That's where this role comes in. As a telephonic RDN, you're the bridge between a member's lab results and their daily choices, catching risks early and steering care before small issues become costly ones. Every call is a chance to move an A1c number, ease a medication side effect, or help someone finally get the DME they've been putting off. The outcomes you drive show up in healthier members and stronger client relationships — which is exactly why this role matters to the business.
What You'll Actually Do
- Assess member health holistically: Conduct comprehensive nutrition assessments that evaluate health status, dietary habits, and clinical risk to build an accurate picture of each member's needs.
- Build individualized care plans: Develop and maintain personalized plans that align with medical conditions, provider recommendations, and evidence-based nutrition guidelines.
- Deliver telephonic counseling and education: Guide members through behavior change conversations that improve self-management and support long-term health outcomes.
- Mine claims data for clinical insight: Use claims processing tools to research paid claim history and uncover patterns that shape a member's care strategy.
- Coordinate access to care: Connect members with primary care providers and specialists, and help secure durable medical equipment and affordable medication options.
- Manage medication and nutrition interactions: Reinforce provider treatment plans by flagging nutrition-medication conflicts and minimizing side effects.
- Drive chronic disease outcomes: Target key health indicators — A1c control, weight management, cardiovascular risk — as part of broader disease management initiatives.
- Collaborate across the care team: Partner with physicians, nurses, and case managers to keep care coordinated and member-centered at every touchpoint.
- Document and report on impact: Track interventions, maintain accurate records, and contribute to quarterly client reporting that demonstrates measurable population health improvement.
- Maintain compliance and readiness: Uphold HIPAA standards and stay audit-ready for URAC and NCQA accreditation through ongoing training and competency development.
What You Bring to Our Team
Education & Experience:
- Bachelor's degree in Nutrition, Dietetics, or a related field from an accredited program (Master's degree preferred)
- Registered Dietitian Nutritionist (RDN) credential
- Minimum of 3 years of clinical nutrition experience
- Prior experience in case management, wellness program coordination, or an equivalent combination of education and experience
- Availability to work evenings as needed
- Ability to obtain and maintain multi-state licensure per business need
Technical Skills:
- Proficiency with claims processing tools and knowledge of ICD-10, CPT, and HCPCS coding
- Working knowledge of Microsoft Word, Excel, and Outlook
- Comfortable operating across Teams, SharePoint, VPN, UM Web/Health Notes, El Dorado, Quick Links, GIAS, ADP, Confluence, and Smartsheet
- Ability to work efficiently across multiple screens with strong typing proficiency
- Phone system and headset operation for high-volume telephonic care delivery

Compensation: This position offers a base salary range of $35-$40 per hour, depending on location, skills, and experience. You're eligible for our full benefits package starting day one.
